언론 인터뷰 - 늦출 수 있다?
늦춘다는 말이 치료제라는 말 이잖습니까? 실제로는 늦추는 효과도 없습니다. 뭘 기준으로 늦춘다고 주장하나요? 서울대병원와서 진료받는 분들은 평소에도 건강관리 잘하시는 부자들이 많으신데 도대체 뭘 대조군으로 비교했냐고요? 더구나 노인들은 잔병도 많아서 일일이 대조군을 매칭하는 것도 거의 불가능합니다.
치매증후군을 이해할 때 기존 가설인 뇌세포 내 단백질 축적에 의해서 발생하는 것 같다는 퇴행성 질환이라는 이해와 뇌세포 내 단백질 축적이 원인인지 결과인지 알수 없다는 최근 이해와 본 연구를 통해 밝혀진 치매증후군은 뇌세포 내 염증 반응의 결과라는 관점에 따라서 각각 가설과 결론이 달라진다.
DDS를 복용하는 나환자에 대한 연구를 시작한 일본의 연구자는 살아있는 사람의 병리와 사후 부검소견을 매칭시키며 연구하는 것 같다. 그들은 치매에 대한 콜린 효과를 중요하게 판단했으며, 항염증물질의 투여가 치매 예방효과가 있다는 것을 알게 되었으나 동시대에 에자이사에서는 기억력을 증진시키는 새로운 물질-아리셉트-을 합성하였다.
그리고 도제로 운영되는 일본 의사 사회는 나환자의 부검 소견을 통계 연구를 통해 결론을 유추하였다. 나환자에서 치매발생률이 적은 원인은 나병때문에 발생이 적었다는 결론을 내렸다.
치매 예방 치료제로서 DDS에 대한 관련 논문을 소개하면 다음과 같다.
1. 치매를 치료하는데 사용될 수 있는 물질에 대한 소개 논문 - 답손(DDS)이 후보 물질이라는 점을 소개하고 있다.

Desferrioxamine, non-steroidal anti-inflammatory drugs, prednisone, dapsone, vitamin E and idebenone are some such agents that are currently under investigation for the preventive or palliative effect in Alzheimer's disease.
2. 부검 소견으로 나환자 중 DDS를 복용하는 나환자군과 결핵에 걸려서 DDS를 복용하지 않는 나환자를 부검을 통해, 뇌세포 내 단백질의 침착 여부를 비교하였다. 결론은 나환자에게 치매가 적은 이유는 나병 자체 때문이라는 것이 결론이다.

Recently, a prophylactic effect of the antileprosy and anti-inflammatory drug DDS (dapsone, 4,4'-diaminodiphenyl sulfone) has been suggested. Lepromatous patients take more DDS (51.9%) than tuberculoid patients (11.5%), however, as the dementia rate of tuberculoid leprosy (17.9%) in those 65-84 years old is similar to lepromatous leprosy (15.9%) in our study, we do not support their viewpoint.
3. 나병의 치료제가 치매의 예방 효과가 없다는 결론을 내린 논문이다.

Therefore, we examined the effect of anti-leprosy drugs on the prevalence of AD in leprosy patients, and also investigated the effect of anti-leprosy drugs on amyloid beta-protein (Abeta)-induced neurotoxicity in vitro. The present study suggests that anti-leprosy treatments do not prevent the onset of AD.
2번과 3번 논문은 부검 후 확실히 알 수 있는 사망원인에 대한 보고가 없다. 사망원인이 무엇이었는지 모르는데 치매와 DDS에 대한 상관 관계를 유추하였다. 통계 처리에서 Selection bias가 없다고 할 수 없는 논문이다.
그래서 연구자를 위해서 치매에 대한 논문을 리뷰하기로 했다.
첫번째 논문은 치매의 시작점을 알 수 없다는 논문이다.

Additionally, AD is associated with the development of amyloid angiopathy (CAA), in which Aβ builds up on the walls of the cerebral arteries, which augments the development of cerebral vascular disease (CVD). Conversely, CVD promotes Aβ deposition and the development of AD by affecting the balance of Aβ production and clearance. However, it remains to be determined whether the accumulation of Aβ is a cause or consequence of AD. The interaction between AD and CVD is a topic of considerable current interest.
치매 발생의 인과관계를 알지 못한다는 논문으로 본 연구에서 allosteric regulation을 핵심 개념으로 설정한 이유이기도 하다.
두번째 논문은 매우 근접한 치매 예방 치료 이론을 제시하나, 경도인지장애와 치매 초기 환자에 대한 분류가 되어 있지 않다. 미국 의료 비용이 대단히 고가여서, 초기 단계 치매 환자군을 확보하지 못한 것 같다. 경도인지장애환자및 초기 치매환자에 대한 치료효과를 개별 코호트로 분류를 하지 못한 점이 안타까운 논문이다.

Epidemiological studies suggest that long-term use of non-steroidal anti-inflammatory drugs (NSAIDs) may protect against Alzheimer's disease (AD). Despite encouraging evidence, all large, long-term, placebo-controlled clinical trials aimed at reducing inflammation in the brain of AD patients produced negative results. More recently it has been shown that some NSAIDs decrease the production of amyloid-beta(1-42) (Abeta42), the major component of senile plaques of the AD brain, and counteract the progression of Abeta42 pathology in transgenic mouse models of AD. The proposed mechanism for this activity is an allosteric modulation of gamma-secretase activity, the enzyme responsible for the formation of amyloid-beta. The inhibition of Abeta42 production is independent from the anti-cyclooxygenase (COX) activity and is related to the chemical structure of the compounds, with some NSAIDs being active (ibuprofen, sulindac, flurbiprofen, indomethacin, diclofenac) and others not (naproxen, aspirin, celecoxib). This could explain the negative results of the large AD trials carried out so far, as they were conducted with compounds (naproxen, hydroxychloroquine, dapsone, prednisone, rofecoxib and celecoxib) that are not able to decrease Abeta42 production. Unfortunately, the use of these NSAIDs in AD is hampered by a significant gastrointestinal toxicity associated with COX inhibition. Thus, new NSAID analogues are being developed with potent and selective inhibitory activity on Abeta42 but with either lack of COX inhibitory activity or reduced gastrointestinal toxicity potential.
한국에는 100년 치매 코호트가 있다. 격리된 환경에서 투약과 치료가 병행되었으므로 매우 유의미한 환자 진료 데이터였다.
<소록도 및 서울 환자 연구>
• 근번 제시한 치매 치료예방법은 뇌세포내 염증반응을 제어하는 기전을 경도인지장애와 치매 초기 치료법을 설명하고 있다. 연구기간이 만10년이었고 변화단계마다 정밀검사를 시행했고 치료의 효과를 증명한 것이 핵심 연구결과이다. 치매는 뇌세포의 염증 반응을 제어할 때 치료와 예방 효과를 거둘 수 있다는 것이다.
연구는 정부와 건강보험에서 치매 등급을 분류하기 시작하고 요양보험으로 지원하고 검사를 보험으로 지원하면서 치매 초기 단계 환자 분류가 가능하게 되면서 연구 환경이 조성되었다. 이제 경도인지장애 환자를 뇌세포 10만개 괴사단계, 100만개 괴사단계, 1000만개 괴사단계, 1조개 괴사단계로 구분하여 염증치료를 준비할 수 있다.
이에 대한 조기진단 검사법을 개발 중이며 개발 가능성이 매우 높다.
현재의 치매 증상개선제를 투여하는 치료법은 이미 뇌세포가 상당수 죽어버린 분들을 대상으로 네종의 증상개선제를 투여하는 치매 마지막 단계의 치료법이다. 마지막 단계 환자를 대상으로 치매에 대한 통계를 내며 사후 부검소견 통계 분석과 유추된 결론은 믿기 어려운 점이 있다. 초기 단계에서 치매를 치료하고자 개발한 뇌세포 내 염증을 치료하는 의약품과 효능을 비교하면 연구오류에 빠지기 쉽다.

특허 내용에 두 가지 물질을 투약하여 치매예방효과가 있다고 기술하고 있다.
그러나 나는 MCI 환자 prospective cohort에서 한가지 물질을 투약하며 관찰하였고, 경도인지장애에서 치매 예방 효과와 치매 이환후 경도인지장애 상태로 회귀되는 과정을 임상에서 증명하였다. 이로써 치매는 아밀로이드 단백질 등이 쌓이는 퇴행성 질환이 아니라 염증 반응에 의해서 발생하는 질환이라는 점을 증명하였고, 치매는 뇌세포에서 다양한 원인에 의해서 발생할 수 있는 염증을 줄여주면 예방할 수 있는 질병이라는 점을 증명하게 되었다.
우리는 2008년부터 증명 연구를 시행하였고, 2015년 미국에서도 DDS를 치매 치료 후보물질로 소개하기 시작하였다. 미국 특허와 논문을 아래에 소개한다.
P2019-0013337_미국 특허공보 US5532219[9582].pdfP2019-0013337_인용참증[9581].pdf하기 인용 논문에서 정상 노인도, 인지장애 노인도, 아밀로이드 단백질은 뇌세포에 축적될 수 있다. 다만 인지장애 노인에게 아밀로이드 단백질 축적 비율이 2-3배 높다. 하기 논문을 참고하면 정상 노인과 인지장애 노인과 치매 노인 사이에 아밀로이드 단백질 축적과 치매 증상 발현 사이에 무엇인가 다른 병리학적 기전이 있다는 것을 추정할 수 있다.

Results The prevalence of amyloid pathology increased from age 50 to 90 years from 10% (95% CI, 8%-13%) to 44% (95% CI, 37%-51%) among participants with normal cognition; from 12% (95% CI, 8%-18%) to 43% (95% CI, 32%-55%) among patients with SCI; and from 27% (95% CI, 23%-32%) to 71% (95% CI, 66%-76%) among patients with MCI. APOE-ε4 carriers had 2 to 3 times higher prevalence estimates than noncarriers. The age at which 15% of the participants with normal cognition were amyloid positive was approximately 40 years for APOE ε4ε4 carriers, 50 years for ε2ε4 carriers, 55 years for ε3ε4 carriers, 65 years for ε3ε3 carriers, and 95 years for ε2ε3 carriers. Amyloid positivity was more common in highly educated participants but not associated with sex or biomarker modality.
Conclusions and Relevance Among persons without dementia, the prevalence of cerebral amyloid pathology as determined by positron emission tomography or cerebrospinal fluid findings was associated with age, APOE genotype, and presence of cognitive impairment. These findings suggest a 20- to 30-year interval between first development of amyloid positivity and onset of dementia.
DDS의 투약이 뇌세포내 염증 감소 효과가 있다는 것이 본 연구의 가설이었고, DDS의 투약이 뇌세포내 단백질 침착을 줄이는 효과가 없다는 것이 일본나환자연구 결론이었다. 두 가설은 치매증후군이 염증 질환이라는 관점과 치매증후군이 퇴행성질환이라는 관점에서 다른 주장이다. 뇌세포내 염증반응에 대한 적극적 치료가 경도인지장애와 초기 치매증후군 치료예방법이라는 것이 연구 결론이다.
