치매 환자의 천국, 네덜란드 호그벡 마을의 비밀
여기 다른 무엇보다 치매 환자의 삶의 질에 주목한 공간이 있다. 치매 환자를 위한 천국이 아닐까 싶을 정도로 잘 설계된 곳. 바로 네덜란드 '호그벡(Hogeweyk)’이다. 호그백은 일명 치매 마을로 불린다. 치매 환자들이 가상의 마을을 이뤄 거주하는 공간으로 설계적 완성도가 매우 높다.
호그벡은 4,500여 평의 부지 안에 커피숍, 슈퍼마켓, 음식점, 공원, 미용실 등을 갖추고 있다. 입주자들은 마을 안에서 일상생활을 누릴 수 있는데 농장에서 채소를 가꾸거나 교회에서 예배를 드리는 것은 물론 미술이나 요리 등 가벼운 취미 생활도 즐긴다. 갇혀있거나 제한되는 것이 중심을 이루던 기존의 요양 병원과는 전혀 다른 모습이다.
그렇다고 치매 마을이 일반 마을과 완전히 똑같은 것은 아니다. 이들이 이용하는 마트에는 가격표가 붙어있지 않으며 미용실에서도 돈을 내지 않는다. 각 상점 등에 배치된 직원은 실제 인력이 아니라 요양 전문 간호사나 간병인, 노인병 전문 의사기 때문이다. 이들은 마을 안에서 치매 환자와 함께 생활하며 환자들을 돕는다. 모든 행동을 간섭하기보다는 최소한의 개입을 목표로 환자가 길을 잃거나 혼란을 느낄 때만 나서는 식이다.
호그벡에는 약 200여 명의 노인이 거주하는데 6~7명씩 한 집에서 생활하고 있다. 일종의 주거 공동체로 입주자들의 특징에 따라 거주 공간이 달라진다. 각 공간은 도시적, 가정적, 기독교적 등의 특징을 지니며 입주자와 보호자는 설문 등을 거쳐 원하는 곳을 선택할 수 있다.
이는 호그벡 마을의 설계자 이본느 반 아메롱겐(Yvonne Van Amerongen)의 배려로, 그녀는 서로 생각이 비슷한 사람들끼리 익숙한 환경에 모여 살아야 스트레스도 덜 받고 갈등도 줄일 수 있다고 봤다. 마을 곳곳에서 돋보이는 특징 역시 그녀의 작품이다.
1992년 요양원에서 간호사로 일하던 이본느는 치매 환자를 위한 좀 더 자유로운 공간이 필요하다고 봤다. ‘치매 환자도 삶의 재미를 느낄 수 있어야 한다’고 생각한 그녀는 병원 경영진에게 치매 마을을 제안했고 이후 정부 지원 등을 받아 2009년 호그벡의 문을 열었다.
호그벡은 현재 치매 돌봄의 성공적인 사례로 손꼽히며 전 세계로 퍼져가고 있다. 프랑스와 스위스, 영국도 이를 본뜬 마을을 건립 중이며 국내 기관에서도 탐방을 가곤 한다.
이러한 반응이 당연하게 느껴질 만큼 호그벡 모델은 분명 매력적이다. 하지만 단순히 호그벡 마을을 따라 하자고 말하긴 쉽지 않아 보인다. 호그벡 이용 가격은 정부 지원금을 포함, 1인당 월 700만 원이다. 평균 대기자는 80여 명으로 보통 1년 정도의 대기 기간이 소요된다. 다소 높은 가격과 시간 부담이 있는 셈이다.
만약 우리나라에도 호그벡 같은 치매 마을이 생긴다면 어떨까? 호그벡은 물리적으로만 따지면 약 255억 원을 들여 3년 만에 만들어졌다. 하지만 우리도 그만큼의 시간과 자본을 투입한다고 해서 같은 효과를 볼 수 있을까?
그 답을 쉽게 내릴 수는 없지만, 호그벡 마을에서 배워야 할 점은 분명해 보인다. 호그벡 마을이 지닌 진정한 가치는 물리적인 환경이 아닌 ‘치매를 대하는 자세’에 있다. 네덜란드는 2004년부터 ‘전국 치매 프로그램’을 수립하여 운영 중이다. 조기 대응이 핵심 과제로 치매 환자가 최대한 일상생활을 유지하도록 돕는 게 목표다.
이는 전문가들 역시 강조하는 부분으로, 치매에 관한 가장 현실적인 대안은 ADL(Activities of Daily Living, 일상생활 능력)을 유지해 삶의 질을 지키는 방안을 찾는 것이다. 실제로 네덜란드는 치매 환자의 80%가 가정에서 생활하고 있으며 그중 절반가량은 혼자서 생활한다. 호그벡 같은 요양원에 입소하는 경우는 24시간 관리가 필요한 중증 치매 환자뿐이다. 이렇듯 호그벡은 단순히 한 요양 시설의 성공이 아니라 네덜란드 정부의 오랜 노력과 민간의 아이디어가 만나 빛을 발한 경우에 가깝다.
The secret of the Dutch village of Haugbeck, a paradise for elderly dementia
Here is a space that focuses on the quality of life of dementia patients more than anything else. A well-designed place that makes it a paradise for people with dementia. It is Haugbeck in the Netherlands. Haugbeck is also known as the village of dementia. It is a space where dementia patients form a village and has a very high degree of design completion.
Haugbeck has a coffee shop, supermarket, restaurant, park, and beauty salon on the 4,500 pyeong site. Tenants can enjoy daily life in the village. They grow vegetables on the farm, worship at church, and enjoy light hobbies such as art and cooking. It is completely different from the existing nursing hospitals where confined or restricted in the center.
However, dementia villages are not exactly the same as ordinary villages. There is no price tag attached to the mart they use, and the beauty salon does not pay. This is because the staff assigned to each store are not actual personnel, but nursing nurses, caregivers, and geriatric doctors. They live with dementia patients in the village and help them. Instead of interfering with all actions, the goal is minimal intervention, and only when the patient feels lost or confused.
About 200 elderly people live in Haugbeck, with 6 to 7 people living in one house. It is a kind of residential community, and the living space varies according to the characteristics of the tenants. Each space has features such as urban, family, and Christianity, and residents and guardians can select the desired location through questionnaires.
This is the consideration of Yvonne Van Amerongen, the designer of the Haugbeck village, and she believes that people who have similar thoughts must live in a familiar environment to reduce stress and reduce conflict. Her work is also a feature that stands out throughout the village.
Yvonne, who worked as a nurse in a nursing home in 1992, saw that he needed more free space for dementia patients. Thinking that “dementia patients should also have fun in life,” she proposed a village with dementia to the hospital's management, and later opened the door of Haugbeck in 2009 with government support.
Haugbeck is currently regarded as a successful case of caring for dementia and is spreading all over the world. France, Switzerland, and the UK are also building villages that mimic this, and domestic institutions also visit them.
The Haugbeck model is certainly attractive enough to take this reaction for granted. However, it doesn't seem easy to say to simply follow the village of Haugbeck. The price for using Hogbeck is 7 million won per person per month, including government subsidies. The average number of people waiting is 80, and it usually takes about a year to wait. It is a rather high price and time burden.
What if there were dementia villages like Hogbeck in Korea? Haugbeck was created in three years, spending about 25.5 billion won physically. But can we see the same effect by investing that much time and capital?
The answer cannot be easily answered, but it seems clear what needs to be learned in the village of Haugbeck. The true value of the Haugbeck village lies in the attitude toward dementia, not the physical environment. The Netherlands has established and has been running the “National Dementia Program” since 2004. Early response is a key task, and the goal is to help dementia patients maintain their daily lives as much as possible.
This is also emphasized by experts, and the most realistic alternative to dementia is to find a way to preserve the quality of life by maintaining ADL (Activities of Daily Living). In fact, in the Netherlands, 80% of dementia patients live at home, and about half of them live alone. Only those with severe dementia who need 24-hour management are admitted to nursing homes like Haugbeck. As such, Haugbeck is not simply the success of a nursing facility, but a case where the long efforts of the Dutch government and private ideas come together to shine.
